  • I developed and restricted while continuing to maintain the entire faculty aftercare system, I created a more efficient productive way to execute all viable aftercare , discharge plans from where a client was to live in a recovery based housing option to their outpatient continuum of plan facility location and setting up that Tx for their continued care.
  • From March 2022- August 2024, I was the only case manager for the entire facility caseload and all their aftercare requirements ,process and discharge planning. handled successfully all the aftercare work and planning per 120 inpatient clients from admission to completion with out fail. I have never had to discharge any client to a shelter placement due to not having a safe Siberia and supportive viable aftercare location for them to reside in recovery. I am proud of that and take pride in all I do for all clients aftercare plan, building their foundation to be as solid as possible so they can build upon it and maintain a solid recovery program.
