Facilitate Workshops in the Re-entry Services Office.

These include life skills and cognitive based learning programs.

Networking and resource gathering for job fairs, presentations, re-entrant focused services.

Coordinate programming with the Re-entry Specialist and Administration to provide insight and preparation for successful transitions of re-entrants.

Participate in Parole Simulations throughout the state at various locations.

Effectively communicate program evaluations and outcomes with supervisory staff.

Assist Re-entrants with connecting with resources and services to allow for a successful transition back into the community.

Develop, implement, monitor and evaluate social plans.

Counsel individuals and/or family members as needed.

Participate in committees/workgroups as assigned Communicate effectively and advocate for individuals.

Prepare reports /provide information in a timely and concise manner.

Participate in the interdisciplinary process as needed and maintain effective working relationships.

Assure consents for supports are completed in a thorough and timely manner Lift, position, transfer individuals, push wheelchairs or other adaptive equipment on level ground or up/down ramps.

Perform CPR, Heimlich or other emergency procedures including evacuations as needed.

Work in any area of the facility and travel as assigned.

Developed and facilitated the Diversion Program.

Organized and facilitated Clinical Review Team Meetings.

Completed numerous Quality Assurance duties including data tracking, policy and procedural reviews, and reviewing reports of abuse submitted to our agency.

Supervised various staff through Child Welfare duties including while they were on-call.

Coordinated Services between families and various community service providers.
