Partnering and collaborating with multiple county and community agencies and landlords to assist in rapidly rehousing the unhoused of Dauphin County who are referred to our program through the Coordinated Entry System (CES). Maintained a case load of as many as 15 – 20 clients who have a multitude of barriers to include (i.e., Mental Illness, Substance Use Disorder, Chronic Health Conditions, impacts of the criminal justice system, lack of a fixed income, etc.). As a RRH Case Manager, I used SMART goal planning throughout the client’s participation in the program.
