  • Provided support in program development, implementation, housing navigation, and intense mobile case management within the ESG Rapid Rehousing Program. Partnering and collaborating with multiple county and community agencies and landlords to assist in rapidly rehousing the unhoused of Dauphin County who are referred to our program through the Coordinated Entry System (CES). Maintained a case load of as many as 15 – 20 clients who have a multitude of barriers to include (i.e., Mental Illness, Substance Use Disorder, Chronic Health Conditions, impacts of the criminal justice system, lack of a fixed income, etc.). As a RRH Case Manager, I used SMART goal planning throughout the client’s participation in the program.

Case Manager responsibilities would be to implement an appropriate treatment plan with the clients, by utilizing evidence-based practices to enhance their overall recovery process while transitioning back into the community.
  • Additional duties were to provide check-ins, breath analysis and drug screens. Review plans with residents weekly and document progress. Incorporating life skills, budgeting, suitable housing, educational, and vocational programs. This approach will help individuals gain financial independence.
